Safe at Home: 10 Older Adults, 10 Solutions

YourPlace Community Foundation is launching Safe at Home: 10 Older Adults, 10 Solutions, a small, hands-on pilot designed to help 10 older adults in the Cass-Clay community overcome practical barriers that make it harder to remain safe and independent at home.

We will work individually with older adults and family caregivers to identify an immediate unmet need, locate an appropriate community resource, and help make the connection. Needs may include food access, transportation, household safety, social isolation, caregiver resources, or difficulty navigating available community services.

The idea is simple: instead of giving someone a long list of resources and hoping they can figure out what to do next, we want to help close the gap between “I need help” and “I found the right help.”

For each participant, we will document the barrier identified, resources or referrals provided, and whether the individual successfully connected with support. What we learn from these first 10 participants will also help us strengthen future aging-in-place and caregiver-support efforts in our community.

A $1,000 Awesome Foundation grant would allow us to turn this focused idea into direct local action and demonstrate how personalized resource navigation can help older neighbors remain safer, more connected, and more independent at home.
